Procedures to Verify FBI Employment

• Draft a letter requesting verification of FBI employment, which includes:
  • Your Name
  • Home Address
  • Phone Number
  • Email Address
  • Social Security Number
  • EOD
  • Date of Retirement
• You will need to send a completed Form DOJ-361 with the letter. Click here for the form.
• Fax the form and the letter to (202) 324-5910.
• If you have questions concerning this, call (202) 324-3333.
• The resultant verification will include your time of other government service, including military service.
• The process takes approximately two weeks.
